肝腫瘍の偽被膜形成に関する研究

研究概要

本年度より、主に転移性肝癌外科切除例を対象に切除標本から腫瘍と非腫瘍組織の採取と保存を開始した、現在のところ、15例(結腸癌肝転移10例、直腸癌肝転移4例、胃癌肝転移1例)を集積した。30例を目標に材料の集積を行う予定である。
標本切除後ただちに標本から新鮮な癌部と非癌部組織を切り出して、4℃のPBS中に浸し、DNAチップによる解析に提出する。癌部には、癌部と非癌部の境界部分が含まれるように切り出す。また切除標本は、遺伝子解析に提出されたものと同じ部分を、ホルマリン固定後にブロックとして切り出し、パラフィン包埋する。それらを従来の染色法(hematoxylin and eosin染色)により病理形態学的に検討する。検討項目は、1)組織学的分化度、2)壊死の割合、3)肝内脈管および神経(門脈、胆管、神経)に対する侵襲の有無、4)癌部と非癌部間の線維組織(偽被膜)の発達の程度(0:まったく発達していない、1:軽度発達;フラーゲン束が10列以内、2:高度発達;コラーゲン束が11列以上)である個々の症例について偽被膜の有無やその発達の程度について、特定の遺伝子との関係やDNAチップによる遺伝子のプロファイリングの結果を比較検討する。大腸癌や胃癌の原発巣がすでに切除されているが当施設でその試料が入手可能な場合や転移巣が原発巣と同時切除の場合には、原発巣の遺伝子発現のプロフィールとの関連についても検討予定である。
