腎細胞癌におけるIGF systemの役割

研究概要

Insulin-like growth factor (IGF) systemは正常細胞や癌細胞の成長や分化に深く関与していることが報告されている。一般的にIGF systemには、IGFs (IGF-I、IGF-II)、type I and II IGF receptors、IGF-binding proteins (IGFBPs)、IGFBP proteasesが含まれIGFはparacrineあるいはautocrineとしてtype I and II IGF receptorsやinsulin receptorに結合することによりmitogenicに働くことが知られている。われわれはマイクロアレイを用いてgene profilingを作成し、腎細胞癌においてIGF familyのIGFBP3とinsulin receptorの強発現を認めることを確認した。そこで、RNase protein assay (RPA)を用いて腎細胞癌と同一患者の正常腎組織とのIGF familyのmRNAの発現を比較し、予後に及ぼす影響を検討した。IGF1、IGF2、type I and II IGF receptors、H19、ALS、insulin receptor、IGFBP1-6の計13遺伝子で検討した結果、12例の腎細胞癌でIGFBP3の100%、insulin receptorの42%、IGFBP1の42%で強発現を認めた。さらに、これらのタンパク質の血清中の発現レベルを検討した結果、IGFBP3は腎細胞癌患者で有意に高かった。平均17ヶ月の経過観察では、これらのタンパク質の発現と予後に有意差を認めず、さらに長期の経過観察が必要と思われた。これらの結果より、腎細胞癌においてIGF familyの測定は診断に有用であり、さらに腎細胞癌治療のターゲット遺伝子になり得ると考えられた。
