研究概要

本研究では、国内は研究代表者が班員を務めている厚生省特定疾患特発性心筋症研究調査研究班が班員によびかけ、国際的には研究代表者が委員を務めている世界保健機構の関連組織である世界心臓連盟心筋症科学委員会、および世界心筋症学会が母体となり、研究代表者が会長となり、心筋症、心不全国際会議を開催し、心筋症研究のメンバーを選定し共同研究グループを設立した。
心筋症の原因ウイルスとして、エンテロウイルス、C型肝炎ウイルス(HCV)について心筋組織からの検出法を確立した。
また、共同研究としてHCVによる心筋障害の評価法を確立するため、HCV肝炎に対するインターフェロン療法の目的に入院した33名を対象とし、心電図、心エコー、201TLおよび1231-BMIPP心筋シンチグラフィを施行した。その結果、正常心機能である慢性C型肝炎患者の一部に心筋シンチグラフィや血漿BNP値で検出される心筋障害が存在し、インターフェロン投与によりこれらの異常改善する事が示された。これらの検査法がHCVによる心筋障害の評価に有用であることが明らかとなった。
さらに、HCV感染者の肝炎発症の有無には、HLA遺伝子多型が深く関与していることから、我々は抗HCV抗体陽性の心筋症患者についてHLA遺伝子解析を行った。その結果、拡張型心筋症ではHLA-A^*1101,肥大型心筋症ではDRB1^*0901,DQB1^*0303に有意な相関を見いだした。
