歯周炎感受性における各種遺伝子多型の寄与率に関する総合研究の企画調査

研究概要

歯周疾患の発症、進行は遺伝的要因が関与しており、中でも歯周病原細菌に対する生体防御機構や免疫応答を司る遺伝子多型の重要性が示唆されている。そして、歯周疾患とこれらリスク遺伝子多型の関連性について、国内外で盛んに検索されるようになってきた。そこで、申請者らは研究施設ごとの解析、検討後、「日本人としての背景集団におけるデータの共有」、「解析手法の共通性」、「臨床基準の共通性」を図り、企画調査を実施した。遺伝子多型の解析内容から、サイトカイン、代謝・酵素、受容体・膜抗原遺伝子多型に分け、発表、討議を行い、以下の知見を得た。
IL-1遺伝子多型については様々な報告があるが、遺伝子多型の人種差もあり、国際的なコンセンサスが得られていないのが現状で、今回の報告でも欧米の報告とは異なるものであった。
代謝・酵素遺伝子多型では小児型低フォスファターゼ症(HOPS)、Papillon-Lefevre症候群(PLS)、歯肉増殖症を対象とした報告がなされた。小児型HOPS患者の臨床症状発現の一因として、組織非特異型ALPのF310L、V3651遺伝子変異の関与が示唆され、易熱性タンパク合成、ALP活性の低下もしくは欠如が見られた。同様に、PLSでのカテプシンC遺伝子変異に伴う酵素活性の欠如も確認された。また、α2インテグリン多型による歯肉線維芽細胞のコラーゲン分解抑制が歯肉増殖症の原因の一つであることも分かった。
更に、HLA-II遺伝子多型が歯周病属細菌に対するT細胞の応答性やTNFα産生に影響すること、FcγR-IIAにおけるリスク遺伝子型について報告がなされた。
以上、歯周疾患感受性遺伝子の特定は複雑、困難であり、今後1.サンプルの大規模的検索、2.複数の遺伝子多型の疾患関連解析3.遺伝子多型と機能解析の実施と国際共同研究の構築が必要だと考えられた。
