新しい病気の治療用アイソトープの開発と利用の推進

研究概要	細胞障害性の強いアイソトープを大量に投与して病気の治療を行う核医学治療(内用療法)は、これまで我が国では法規制の整備が不十分なこともあり、甲状腺細胞がヨード(I)を取り込むことを利用した甲状腺疾患に対するI-131治療以外ほとんど行われていない。そこでI-131標識化合物、褐色細胞腫に対するI-131-MIBGおよびB細胞悪性リンパ腫に対するI-131標識抗CD20抗体などI-131標識物を用いる核医学治療について検討した。 I-131-MIBG治療に際しては本学倫理委員会の許可を得るとともに患者から文書にて同意を得て行った。すでに遠隔転移を有する3例の悪性褐色細胞腫患者に対してI-131-MIBG治療を行い、一過性の高血圧、悪心などの消化管症状以外に重篤な有害事象は認めていない。 RI標識抗体は副作用の少ない腫瘍特異的な治療法として臨床応用できると期待されている。抗体の標識に用いる治療用RIとしては、イットリウム(Y)-90が最も優れている。抗体とY-90との標識は簡便に行うことができ、Y-90標識抗体の尿中への排泄率は10%以下で、生体内でも安定であることが確かめられた。Y-90はDTPAを介して標識することができるが、β線のみを放出するため、γ線を放出するインジウム(In)-111を用いて検討した。標識方法を改善することにより肝臓、腎臓への生理的な取り込みを減少させ、腫瘍/正常組織比を高くすることにより治療効果を増強することができた。ストロンチウム(Sr)-90、サマリウム(Sm)-153標識化合物による転移性骨痛の治療も開発されており、Y-90の基礎研究、臨床応用も活発になりつつある。今後RIを用いる新しい病気の内用療法が普及するものと期待される。
