虚血性心疾患における免疫学的機序の研究

研究概要

本研究では炎症において中心的役割を旺たすT細胞に注目し、虚血性心疾患患者における活性化について調べた。活性化されたT helper cellとしてTh1 cellとTh2 cellがある。Th1 cellが産生するinterferon-gammaとTh2 cellが産生するinterleukin-4といった細胞内サイトカインを染色することでT helper cellがTh1 cellまたはTh2 cellのいずれへ活性化しているのかがわかる。虚血性心疾患患者から採血した検体を二つに分け一つは活性化したT helper cellをフローサイトメーターにかけて細胞内サイトカインのinterferon-gammaとinterleukin-4を染色した。interferon-gammaとinterleukin-4の細胞内発現の程度を不安定狭心症患者、冠攣縮性狭心症患者、安定狭心症患者および健常者で比較した。検体の残り一方は円沈分離して血清を取り出し、血清中Th1系蛋白を測定した。Th1系蛋白としてはinterferon-gammaの他にCD 40 ligandおよびinterleukin-18などがあるが、その血清レベルを測定し前述の各患者群で比較した。その結果、虚血性心疾患のなかで不安定狭心症と冠攣縮性狭心症の患者のT細胞では健常者に比べinterleukin-4の産生亢進はなくinterferon-gammaの産生亢進が認められ、これらの患者ではT細胞のTh1 cellへの活性化があることが分かった(Circulation 2003 in press)。また有意狭窄を有する安定狭心症患者のT細胞では冠動脈病変の多少に関わらずTh1 cellへの活性化は認めなかった。また、血清中のTh1系の蛋白質としてCD 40 ligandおよびinterleukin-18について調べた結果、不安定狭心症や冠攣縮性狭心症患者では血中蛋白質もTh1系の蛋白質が健常人に比べ多くなる傾向が認められた。これらにより不安定狭心症患者だけでなく冠攣縮性狭心症患者においてもT細胞のTh1 cellへの活性化があるということが証明されたことになる。
