虚血性心疾患における免疫学的機序の研究

研究概要

本研究では炎症において中心的役割を果たすT細胞に注目し、虚血性心疾患患者における活性化について調べた。活性化されたT helper cellとしてTh1 cellとTh2 cellがある。Th1 cellが産生するinterferon-gammaとTh2 cellが産生するinterleukin-4といった細胞内サイトカインを染色することでT helper cellがTh1 cellまたはTh2 cellのいずれへ活性化しているのかがわかる。虚血性心疾患患者から採血を行い、そのhelper T cell内のinterferon-gammaとinterleukin-4を染色した。interferon-gammaとinterleukin-4の細胞内発現の程度を不安定狭心症患者、冠攣縮性狭心症患者、安定狭心症患者および健常者で比較した。その結果、不安定狭心症と冠攣縮性狭心症の患者のT細胞では健常者に比べinterleukin-4の産生亢進はなくinterferon-gammaの産生亢進が認められ、これらの患者ではT細胞のTh1 cellへの活性化が生じていることが分かった(Circulation 2003)。また、血清中のTh1系の蛋白質としてCD 40 ligandおよびinterleukin-18について調べた結果、不安定狭心症や冠攣縮性狭心症患者では血中蛋白質もTh1系の蛋白質が健常人に比べ多くなる傾向が認められた。また、膠原病患者ではリュウマチ性関節炎のようにTh1に活性化されているものとsystemic lupus erythematosusのようにTh2に活性化されているものとがあり注目して同様の検討をしてみた。その結果、膠原病患者で虚血性心疾患を有するものと有さないものとで比べてみると、虚血性心疾患を有する患者ではTh1へ活性化されやすく、虚血性心疾患のうちでは冠攣縮性狭心症患者が多いことが分かった(Circulation Journal 2004 in press)。さらに、経皮的冠動脈インターベンションにより患者のhelper T cellはTh1 cellへ活性化してしまうこと、事前にstatinを投与することでその活性化を抑制できることも分かった(American Joural of Cardiology 2004 in press)。
