研究概要	球脊髄性筋萎縮症(SBMA)は、アンドロゲン受容体(AR)遺伝子のCAGリピートの異常延長を原因とする。成人男性に発症する緩徐進行性の遺伝性運動ニューロン疾患である。その病因として、核内に集積した変異タンパクが転写因子などの機能を阻害し、脊髄および脳幹の運動ニューロンの機能障害が生じることが考えられている。97CAGを有する全長ヒトARを導入したトランスジェニックマウス(Tg)では、神経症状が雌に比べ雄において重篤かつ急速に進行し、かつ変異ARの核内集積が雄において雌より顕著にみられた。雄Tgに去勢あるいはLHRHアナログの投与を行ったところ血清テストステロン濃度の低下に伴い変異ARの核内移行が抑制され、運動障害は著しく改善した。ARは通常細胞質でheat shock protein (HSP)などと複合体を経形成し不活化されているが、リガンドとの結合によりHSPから離れて核内へと移行する。去勢やmRHアナログは、テストステロン分泌を抑制することでリガンドに依存した変異ARの核内移行を阻害し、神経細胞の変性を抑止するものと考えられる。LHRHアナログはすでに前立腺癌などで使用されている薬剤であり、平成15年からSBMA患者に対するLHRHアナログの臨床試験を実施している。他方、HSPなどの分子シャペロンは構造異常のあるタンパクをrefoldする作用が知られている。HSPによるSBMAの治療を検討するため、HSP70の高発現マウスを上記Tgと交配した。その結果、HSP70の高発現により変異ARの核内蓄積が抑制され、運動機能や寿命などの表現型も改善した。これは、変異ARのrefoldingによる凝集抑制に加え、変異ARの分解が促進したためと考えられる。HSP70の発現を誘導する薬剤の治療効果が期待される。
