早期の癌に対する標準的放射線治療方法確立のための研究

研究概要

画像診断の進歩により早期の癌が高率に発見されるようになり、また高齢化社会の進行に伴い放射線治療の役割が増している中で、治癒の可能性の高いこれら早期の癌に対する標準的放射線治療方法(照射野、照射方法、線量分割法、総線量、小線源との併用、化学療法との併用など)は確立していないのが現状で、本研究はその確立を目指すものである。本年度は研究分担施設等の過去のデータを集積、解析すると共に、研究の方向性を定めた。I期肺癌に対して定位放射線治療を行った241例の解析では、CR23%、PR62%で、5年生存率は85%であった。詳細な解析をさらに行い、定位放射線治療の許容される精度、1回線量、および総線量について推奨を行う。またT2N0M0肺癌に対して、肺門部照射および化学療法併用の必要性についてphase III trialを計画する。I、II期悪性リンパ腫はすでに155例集積しており、5年生存率は71.5%であり、照射方法、化学療法について解析を行っているが、さらに集計を継続する。また75歳以上の早期悪性リンパ腫に対して化学療法併用が必要か否かのphase III trialを計画する。食道表在癌に対しては、すでに標準的治療方法の推奨を行っており、現在その妥当性について検証中である。今後はさらに照射野および小線源治療併用に関してそれぞれphase III trialを計画する。I、II期中咽頭癌については文献で報告された253例の解析を行った、早期下咽頭癌については48例を集積したが、さらに集積を継続する。早期頭頸部癌に関してはIMRTによる唾液腺保護のためのphase I trialを計画する。早期乳癌に対する乳房温存療法については、照射が必要な症例の選別に関する詳細な研究を行い、またboost治療の妥当性について検討を行う。また早期前立腺癌に対する標準的IMRT確立のための研究を行う。
