インスリン様増殖因子(IGF)と大腸前がん病変に関する分子疫学的研究

研究概要

大腸発がんにおけるインスリン様増殖因子IGF-Iと結合蛋白IGFBP-3の役割を明らかにする目的で、大腸内視鏡検査が非選択的に実施されている職域集団において、保存試料を用いて、血中IGF-I、IGFBP-3およびCペプチドの測定をおこなった。1997年1月〜2002年3月の期間の受診者3408名のうち、血液提供を受けた3370名について血中IGF-I、IGFBP-3およびCペプチドの測定を終了した。予備的検討として、IGF-IおよびIGFBP-3と関連する生活習慣要因ならびに血液生化学検査値との関連性を検討し、body mass indexならびに腹囲・臀囲比がIGF-IおよびIGFBP-3と強く関連していることが明確になった。しかし、高度肥満者ではIGF-I、IGFBP-3ともに低下傾向が見られた。
現在、大腸腺腫症例599例と大腸内視鏡異常なしの対照例1466例について、耐糖能、Cペプチドなどの影響を考慮してIGF-IおよびIGFBP-3との関連性を検討している。腺腫の大きさ別・部位別の解析も計画している。また、IGFと関連する遺伝子多型の解析を開始した。2002年4月以降も調査を進めており、約2500名の生活習慣要因ならびに医学検査のデータが追加され、血液試料も保管している。
