脳腸相関におけるストレス関連ペプチドCRHの役割

研究概要	本年度の研究目的は消化管刺激により脳腸の特定部位で神経伝達物質、特にストレスの鍵物質であるcorticotropin-releasing hormone(CRH)が放出され、中枢機能ならびに消化管機能を変化させるという仮説を検証し、脳腸相関の物質的基盤を解明することである。まず、過敏性腸症候群(IBS)患者10例、健常対照者10例に対し、大腸内視鏡にて下行結腸にカテーテルを挿入し、バロスタット法と消化管内圧測定法により、消化管機能を測定しながら大腸電気刺激ならびに伸展刺激を行った。CRHの拮抗薬α-helical CRH 10μg/Kgを点滴静注し、薬物投与前後で反応差を比較した。電気刺激により、IBS群の大腸運動係数が有意に増加し、10mmHgの低圧負荷時の大腸容量が減少した。α-helical CRH投与により、これらは抑制され、対照群の大腸容量が増加した。IBS群では大腸電気刺激ならびに伸展刺激により、不安尺度が上昇したが、これもα-helical CRH投与により抑制された。次に健常者の下行結腸に大腸伸展刺激を加え、サイクロトロンで合成した核種H_2^<15>O生理食塩水を経静脈的に投与し、positron emission tomography(PET)を行った。大腸伸展刺激により、視床、前帯状回、前頭前野の局所脳血流量増加が認められ、同時に内臓知覚と不安が誘発された。さらにラットのバロスタット法を確立し、外腹斜筋筋電図を測定しながら大腸伸展刺激を行った。大腸伸展刺激によって、筋電図spike数が有意に増加した。この時、高架式十字迷路で検出される不安行動が誘発された。これらの反応はCRHの拮抗薬α-helical CRHならびに選択的CRH-R_1受容体拮抗薬で抑制された。以上から、脳腸相関の病態がCRH神経伝達の修飾により制御できることが示唆された。
