関節リウマチにおける自然免疫系の解明とその制御

研究概要	1.RA滑膜における微生物感染に対する自己防御から自己破壊(自己免疫)応答への進展機序の解明 (1)滑膜組織におけるToll-like receptor発現の検討:人工関節置換術で得られた関節リウマチ患者の滑膜組織を使用して、免疫組織染色を行った。 (結果)滑膜細胞にはTLR4、浸潤リンパ球にはTLR1およびTLR5の染色がみられた。 (2)滑膜組織、関節液中のNK細胞の表面抗原の検討:人工関節置換術で得られた関節リウマチ患者の滑膜組織に浸潤しているリンパ球をコラゲナーゼ処理にて分離した。また、関節リウマチ患者の関節液をスプラーゼ処理し、細胞成分を分離した。各患者の末梢血も同時に採取した。これらの細胞中のNK細胞の表面マーカーをフローサイトメーターを使用して、3カラー解析した。解析した表面抗原は、CD160、CD161、CD244、CD2、CD69、NKG2D、NKp46、NKp30の8分子である。 (結果)NK細胞上のCD160分子、NKp46分子、NKp30分子の発現が、末梢血に比べ有意に減弱していた。減弱していた分子は、細胞傷害性NK細胞(善玉NK細胞)に多く発現されている。これらの分子の発現低下がみられたことから、滑膜組織、関節液中には、サイトカインを産生する活性化NK細胞(悪玉NK細胞)が増加している可能性が示唆された。 2.動物モデルにおける自然免疫系の重要性の検討;NK細胞、好中球に含まれるグランザイムBの関節炎発症における役割を解明するため、グランザイムBノックアウトマウスを使用、タイプIIコラーゲンに対する抗体で関節を誘導した。 (結果)投与4週までの観察では、ワイルドタイプとノックアウトマウスとでは、関節炎誘導率に差はみられなかった。
