粒子線を軸にした難治癌の診断と治療法の開発

研究概要	まず,陽子線・炭素イオン線の臨床応用の前段階として,in vitroよびin vivoにて生物学的データの収集を行った.in vitroでは細胞照射実験により,in vivoではマウス腸管照射およびマウス皮膚照射により,各ビームの拡大ブラッグピーク内の異なる深さでの生物学的効果比を求め,臨床応用時の生物学的等価線量算出根拠とした. その後,兵庫県立粒子線医療センターにおいて,陽子線治療の臨床治験を開始した.これは陽子線治療装置の安全性・有効性を検討するためのもので,頭頚部・肺・肝・前立腺癌の患者30例を対象とした.内訳は頭頚部癌4例(脊索腫術後再発,上顎洞,篩骨洞,顎下腺),肺野型肺癌5例,肝癌5例(塞栓術後再発を含む),前立腺癌16例(ホルモン療法後を含む)であった.照射線量および分割回数はこれまで他施設から報告されている方法を参考に設定した.肺・肝癌に対しては呼吸同期照射を実施した.治療終了後1ヶ月時に効果判定を行い,治療開始90日目までの急性反応(米国国立癌研究所共通毒性基準第3版にて判定)を評価した.一次効果は良好で,急性反応も軽微であった.装置の大きなトラブルも経験しなかった. また,陽子線が組織に照射されたときに生成されるpositron emitterに着目して,PET (positron emission tomography)が陽子線照射範囲の検証に有用かどうかを検討した.陽子線治療直後の患者のPET画像を撮像し,治療計画における線量分布と比較したところ,PET画像における高輝度領域と治療計画における照射領域はよく一致した.つまり,陽子線治療直後にPET画像を撮像すれば,治療計画通りに照射されているかどうかを容易に検証することができるということが示された.
