内臓リーシュマニア症の早期発見・早期治療のための基礎的応用的研究

研究概要

(1)簡便かつ非侵襲性の新たな免疫診断法の開発
尿を検体とした凝集反応による新たな診断法を開発することができた。内臓リーシュマニア症の血清診断法としてLeishmania donovaniの染色虫体を用いた直接凝集反応(Direct Agglutination Test, DAT)があるが、これにenhancing antibodyとして抗ヒトIgGを加えることにより、尿中の微量の抗体を検出できるようにした(尿DAT)。この方法の感度は90.7%、特異性は96.4%と、以前開発した高感度の尿ELISA法とほぼ同じであった。(Am J Trop Med Hyg 2004)。
(2)尿ELISA法、尿DAT法の流行地への応用と評価
この簡便な尿DATならびに尿ELISA法を内臓リーシュマニア症の流行地からの検体に実際に使用し、感度、特異性、使い勝手を評価する目的で、平成14年12月にバングラデシュの内臓リーシュマニア症流行地であるRajshahi Districtにある4つの村を訪問し、血清860検体および尿952検体を採取した。現地では、血液より尿の採取のほうが住民の協力が得やすく、はるかに容易であることがわかった。市販のイムノクロマト法による血清中の抗体検査では39例(4.5%)が陽性となった。尿ELISA法による尿中の抗体検査、新たに開発した尿DAT法による尿中の抗体検査、従来のDAT法による血清中の抗体検査は、現在進行中である。
(4)尿ELISA法改良のためのリコンビナント抗原の作成
2次元泳動により分離されたL.donovaniのpromastigoteの抗原成分の解析から、calreticulinが有用な診断用抗原になることが推定された。現在、リコンビナントを作成中である。
本研究は、愛知医科大学およびBangladesh Medical Research Councilの倫理委員会の承認を得た。
