閉塞性動脈硬化症・ビュルガー病に対する自家骨骨髄細胞移植

研究概要

1.重症虚血性末梢血管疾患では血管内皮機能が著しく傷害されていることが予想される。内皮機能障害がさらなる動脈硬化を促進して悪循環に陥っていることが推定される。従って末梢血管疾患での内皮機能を評価し、さらに移植療法が内皮機能改善効果を有するか否かを確認することは重要である。我々は、21例の重症虚血性末梢血管疾患に対し自家骨骨髄単核球細胞移植を施行した。骨髄単核球細胞移植により虚血部血流量の増加、ABIの改善、組織酸素量の増加、自覚症状の改善等が認められ、障害された血管内皮機能も改善していることが明らかとなった(Circulation.2004)。新生血管の血管機能を明らかにして骨髄細胞移植の有用性を血管代謝の側面より評価した。
2.重症虚血性末梢血管疾患に対する自家骨骨髄単核球細胞移植の有用性は明らかであるが、実施に際し大量の骨髄採取を必要とするため採取時に全身麻酔あるいは腰椎麻酔を必要としてきた。患者負担軽減のため少量の骨髄液を採取して血管内皮細胞に分化すると考えられる骨髄間葉系幹細胞を取り出し培養増殖して移植に用いる試みを行っている。
1)ラット下肢虚血モデルにおいて間葉系幹細胞移植により血管新生を来すことを証明し、新生血管およびその末梢組織の血管内皮機能を測定した。血管内皮機能が傷害されたモデルにおいて、骨髄幹細胞移植および間葉系幹細胞移植による血管新生療法を施行し、血管新生が良好に発現し、また発現された新生血管およびその末梢組織血管における血管内皮機能の改善を確認した(投稿中)。
2)非常に効果的で安全な骨髄間葉系幹細胞増殖法を確立しており、同時に、実用化に向け自動培養増殖装置を開発し稼動させている。5mL程度の骨髄採取であれば、約2週間で一般的に必要とされる10^9個の細胞に急速増殖可能なことをヒト骨髄間葉系幹細胞においても確認した。臨床応用に向け準備中である。
