腎疾患関連機能遺伝子"メグシン"の分子機構解明

研究概要

セリンプロテアーゼインヒビター(セルピン)変異体は立体構造の変化から小胞体(ER)内で分子同士が重合しやすくなり、細胞死を招くことが知られている(セルピノバシー)。本研究で我々は、メサンギウム細胞に高発現する新規セルピン"メグシン"を高発現させたラット(メグシンTgラット)を作製したところ、腎臓においてセルピノパチーと類似の病像を呈することが示されたので、その病態の解析を行った。
全身にメグシンを高発現したメグシンTgラット(ホモ)では、興味深いことに、腎臓においてのみ多量のperiodic acid Schiff(PAS)陽性沈着物が検出された。電顕及び免疫電顕にてそのPAS陽性沈着物は肥大した粗面ER内に局在し、メグシンを含むことが示された。腎臓においては、メグシン沈着物は糸球体上皮細胞、遠位尿細管、集合管に認められた。それら細胞ではER肥大に伴いERストレスマーカー(ORP150)の発現亢進が認められ、メグシン沈着物によるERの形態的、機能的異常が確認された。さらに一部の遠位尿細管上皮細胞ではアポトーシスが誘導されていた。また病理変化に伴い、腎機能障害(BUNと血清クレアチニン値の上昇、蛋白尿)も認められた。メグシンTgラット(ホモ)は生後3週にはメグシン沈着物が光顕レベルで検出され、5-6週令から腎障害を発症し、10週令までにはほぼ100%が致死に至る。ちなみにヘテロメグシンTgラットにおいても同様のメグシン沈着物が検出されるが、その程度は軽度で10週令までに腎障害を発症しない。
以上の成果から、高発現させたメグシンはセルピノバシーを発症し、腎障害、膵障害を呈することが明らかになった。さらにこの疾患モデルはERストレスと上皮細胞障害の関連性を示唆する新しいモデルと考えられる。
