高磁場及び超高磁場MRI装置を用いた、脳腫瘍患者における神経線維束の描出

研究概要

平成15年度は既存設備である1.5Tの臨床診療用MRI装置(Siemens社製Magnetom Symphony)、3Tの臨床研究用MRI装置(Siemens社製Magnetom Trio)のそれぞれを使用して正常ボランティア(6名)の拡散テンソル画像撮影を行った。画像解析はJohns Hopkins大のMoriらが開発した拡散テンソル画像解析及び白質神経束描出(tractography)用のソフトウェアDTI Studio(Mori et al.Ann Neurol 2002;51:377-380)を用いた。結果は画像の歪みが原因で必ずしも良好な拡散テンソル画像、Tractographyを得る事は困難であった為、画質向上を図る目的で1.5T、3TのMRI双方にparallel imaging法と頭部専用高感度受信コイルを導入した。正常ボランティア(6名)の撮影において双方とも歪みの少ない良好な拡散テンソル画像の撮影が可能となり、3TのMRIが本来持っている高い高磁場特性による高い信号・雑音比を持った画像がより有効に利用できるようになった。現在、正常ボランティアの数を増やして更に撮影件数を重ねており、学会発表及び論文投稿の準備中である。
脳腫瘍などの脳占拠性病変症例においては特に3TのMRIの利点(前述)を生かして術前tractographyを作成し、手術時における電気生理学的手法による、錐体路などの白質神経路の推定と術前tractographyの対比参照、術後のtractographyと術前の対比を開始している。現時点では少数経験ではあるが、Tractographyにより高精度の切除範囲決定と術後機能温存が可能となってきている。
