研究概要

平成15年度において、優位大脳半球の側頭葉下面における言語関連中枢の同定に関する研究を行い、次のような成果が得られた。
1)優位大脳半球同定に関する検査手法の確立
言語優位半球の同定において、最も信頼性の高い検査であるWada testに従来使用されていたアモバルビタールの製造中止に伴い、アモバルビタールに替えて、本邦で初めて麻酔薬のプロポフォールを用いてWada testを実施し、プロポフォールがWada testにおいてアモバルビタールと同等の安全性と有効性を有することを証明し、実施手法を確立した。
2)優位半球側頭葉下面の脳機能マッピングによる言語機能の解明
難治てんかん患者の術前検索の一環として、読み、書き、音声言語理解の要素を含む多様な言語課題を用いて側頭葉下面の皮質電気刺激による検索を行った結果、次のような知見を得た。(a)優位半球側頭葉下面は視覚言語処理過程と、聴覚言語処理過程の一部に関与している。(b)視覚言語処理過程に関しては、側頭葉下面内に漢字処理に関与する領域と仮名処理に関与する領域が別々に存在する。(c)漢字の認知処理過程と図形・絵の認知処理過程は一部共通要素を持つ。
3)優位半球側頭葉下面の事象関連電位による言語機能の解明
漢字・かな文字単語の音読と物品の絵の呼称を中心とする課題を用いて側頭葉下面に留置した慢性硬膜下電極から事象関連電位を記録して解析を行い、現在、脳機能マッピングの結果との比較検討を進行中である。
4)術前術後での神経心理検査結果の検討
皮質電気刺激結果の評価に基づいて、機能温存による難治側頭葉てんかん外科治療を行った症例について、術前の神経心理検査と術後1年にわたる追跡調査を実施し、術前術後の比較検討を実施中である。
