小児脊椎分離症におけるすべり症発生機序の解明

研究概要	小児の脊椎分離症に続発するすべり症の機序には脊椎終板部の成長軟骨障害が関与していることを臨床研究および生体力学的・組織学的研究から示してきた。平成15年度はさらにその詳細を時間経過を追って組織学的に検討した。その結果、ラットモデルでは分離作成後、成長軟骨板が肥大軟骨層を中心に徐々に変化を起し、一週間後には破綻に至る過程が明らかとなった。この過程で成長軟骨板の前方部に成長軟骨の過形成または骨化障害と思われる現象を観察したため、現在検索を進めている。分離症そのものの発生機序についても検討を進めた。臨床的検討では保存的治療による分離癒合の検討から、癒合に対しては分離部の状態、分離高位が最も重要な因子であることが分かった。さらにスポーツ種目により分離発生側、分離形態に特徴があることが分かった。この臨床的な分離形態の観察は生体力学的検討結果とよく符合した。すなわち、野球など回旋運動が多い種目では利き手と反対側に分離が多いが、三次元有限要素法を用いて行った回旋、伸展運動での腰椎内のストレス分布の検討でも回旋により分離部にストレスが集中することが明らかとなった。癒合に最も重要な因子である分離部の状態をより詳細に評価するためにMRIを用いた。この結果、CTなどのレントゲン評価で分離が明らかとなる前の椎弓根部骨髄内の信号変化を捉えた。三次元有限要素法では、分離部に近いストレスが椎弓根部にかかることが明らかとなり、椎弓根部のMRI異常像を初期変化として評価することの妥当性が証明された。
