抗癌剤耐性を示す難治性口腔扁平上皮癌に対する治療戦略

研究概要

口腔扁平上皮癌を初めとする頭頸部扁平上皮癌に対してシスプラチンの誘導体であるカルボプラチン(CBDCA)を中心とした化学療法剤が使用され、ある程度の臨床的な効果が得られているが、依然として抵抗性を示す症例もあり、その対策が望まれている。Tumor necrosis factor(TNF)-related apoptosis-inducing ligand(TRAIL)はTNF受容体ファミリー蛋白質に属する分子であり、ある種の腫瘍細胞にアポトーシスを誘導するが、正常細胞には何ら影響を与えないことが知られている。扁平上皮癌細胞を抗癌剤エトポシドあるいはTRAILで処理すると細胞死が誘導されるが、エトポシドで予め前処理すると、TRAILに対して高い感受性を示すことを明らかにした。この効果は多剤耐性を示すBCl-X_L高発現扁平上皮癌細胞にも認められたことより、難治性悪性腫瘍に対する有効な治療であると期待できる。次に、効率的なTRAIL作用を期待して、レトロウィルス発現系を用いて、抗癌剤前処理によりレトロウィルスTRAILが有効に細胞死を誘導することを示し、遺伝子治療応用への道を開いた。また、N末端切断型Bax-αは野生型Bax-αよりも強力なアポトーシス誘導能を保持し、BCl-x_L高発現悪性腫瘍細胞株にも細胞障害性を示すことを明らかにした。以上の事実により、TRAIL/抗癌剤併用療法やN末端切断型Bax-αは抗癌剤に耐性を示す難治性悪性腫瘍に対して有効であることがin vitroで示された。次年度は有効な発現系の開発,動物レベルでの有効性を検討していきたい。
