三叉神経領域疼痛における上位中枢系の関与特異性と病態解明に関する研究

研究概要

口腔顔面領域の知覚は身体他部位に比べ、臨床的に心理的影響が大きいとされているが、詳細は明確にされていない。今回我々は健常人を用いて三叉神経領域として舌神経支配毎域である舌尖と比較部位として手背の温度感覚を測定し、その閾値と精神状態の関連性を検索した。対象者は健常人ボランティア55名(男性27名、女性28名)中央値27歳四分位偏差1.5歳であり、文書にて同意を得た上で精神科医による構造化面接(Mini International Neuropsychiatric Interview ver5.0.)と心理検査(SDS:Self-rating Depression Scale)感覚検査には手背と舌尖に圧感覚能検査(SW知覚テスター^<【○!R】>)温度覚検査(痛覚計^<【○!R】>)、[温覚感受最低閾値、熱痛覚感受最低閾値]、電流知覚閾値検査(neurometer^<【○!R】>)を行いロジスティック解析を用いて各種感覚に関連する心理的要因の検出を試みた。舌尖において圧感受能検査において加齢と閾値の低下に関連性が認められ、SDS得点の上昇とCPT2000Hz、温覚、熱感覚感受閾値の低下に関連生が認められた。よって舌尖の感覚閾値の低下は手背と比較してうつ状態尺度であるSDS得点の上昇との関連性が認められた。このごとからも口腔顔面領域の知覚が他の部位よりも精神状態の影響大きいこと可能性が示唆された。今後はこれらのことを踏まえてfMRIを用いて口腔顔面領域の知覚における情動との関連性の検索を行いたい。
