研究実績の概要 |
The objectives of research in FY 2017 are to explore the similarities and differences of communication in patient handoff between countries, and to develop more effective communication protocols. Corresponding to the objectives, summary of achievements is as follows: (1) A survey on nurse views of patient handoff was conducted in over 30 Japanese general hospitals. A six-dimension structure forming performance assessment was acquired for handoff. Based on the structure, Japanese nurses perceived high acknowledgements to overall quality of patient handoff and patient safety, but pointed out problems in efficiency and environment. Comparing to six years ago, nurses’ evaluations were significantly higher. (2) Comparing Chinese and Japanese nurses, Chinese nursing staff expressed more positive perceptions to underlying conditions related to safety and quality of patient handoff, and higher frequency of sufficient information transfer. (3) Semi-structured interviews and focus group discussions were conducted in two Chinese hospitals. Based on the results, new standard communication protocols were developed for patient handoff from ambulance to hospital, between departments in the hospital, and nurses’shift changes.

理由
The main reason for the delay is the original plan is to investigate patient handoff only in Japan, but the applicant added study in Chinese hospitals, thus it is difficult to catch the schedule in two countries. The research plan and current status are stated as follows: (1) Plan: To conduct Japanese hospitals to cooperate in the questionnaire survey about patient handoff focusing on communication. International comparison could be made to investigate weak and strong points in each country and learn from each other. The Current status: A questionnaire survey in over 30 Japanese hospitals has been conducted and comparisons have been made. (2) Plan: To implement the developed protocols in the hospital. Current status: Protocols for Chinese hospitals have been developed and suggested. However, as the questionnaire survey responses are still under analysis, protocols for Japanese hospitals have not been developed and implemented yet. (3) Plan: To device an effective assessment tool for handoffs. Current status: Based on the literature review results in FY 2015, candidate indicators for handoff evaluation have been proposed such as safety, efficiency, information sufficiency, information accuracy and satisfactions of sender, receiver and patient.

今後の研究の推進方策 |
As the project could not be finished in FY 2017, there are several steps for the following year as: (1) As the new communication protocols for patient handoff have been proposed to Chinese hospitals, to evaluate the outcomes of the new protocols after implementation by the developed assessment tool. (2) To analyse the Japanese questionnaire survey responses, to conduct semi-structured interviews and focus group discussions with physicians, nurses and risk managers in Japanese hospitals. (3) To collect the patient handoff incident reports from the surveyed Japanese hospitals. To analyse the reports using the developed taxonomy system. Based on the results of questionnaire survey, interview, focus group discussion and incident analysis, to propose new communication protocols for Japanese hospitals. (4) To implement the proposed protocols in Japanese hospitals and make evaluation using assessment tool.

次年度使用額が生じた理由 |
Because of one year maternity leave, the total progress of the research is still a bit behind the original plan. This is the reason why the funding has not been used up. Therefore, one year will be extended for this research project. The usage plan is as follows: First, computer related consumables are necessary. Then, additional copies of papers from international journals and books will be bought. In addition, to make interviews and discussions with healthcare professionals, expense for domestic travels are needed. Finally, the applicant is willing to attend international conferences and have meetings with foreign scholars. Therefore, expenses for travels abroad are necessary.
