研究実績の概要 |
We conducted multiple preliminary experiments among healthy subjects to select the optimal exposure parameters for whole-body vibration (WBV). Overall, the findings of preliminary experiments revealed that WBV generated at vibration frequencies of 15 Hz, 20 Hz, 25 Hz and 30 Hz with peak-to-peak displacements of 2and 4 mm were able to cause significant increases in peripheral circulation and vibrotactile perception in the lower extremity without altering heart rate variability. Furthermore, moderate improvements in functional mobility and dynamic balance were also observed. In parallel, we also conducted a systematic review and meta-analysis on the intervention and response characteristics of whole-body vibration exposure on peripheral circulation. Accordingly, although frequently used in previously published studies, vibration frequencies of 25 Hz and 30 Hz with higher peak-to-peak displacements (for example, 4 mm as used in the preliminary experiments) far exceed the International Standard (ISO) recommended A(8) value. In line with the results of the preliminary experiments, review study and recommendations of International Standard (ISO 2631-1), the following vibration exposure conditions have been selected for use in the final experiments: vibration frequencies of 15 Hz, 20 Hz and 25 Hz with a peak-to-peak displacement of 2 mm each.

理由
The research grant was approved late by half year. Since then, we have procured necessary equipment relevant to different tests and arranged the experiment room. In the meantime, we performed a review study on the intervention characteristics of various types of whole-body vibration and response patterns in various physiological parameters. In line with the findings of that review, we conducted multiple experimental sessions to select the optimal parameters for vibration to be used in our study. Later, we conducted the preliminary experiments to evaluate the feasibility and effectiveness of those selected parameters among healthy subjects, according to purposes of our study. Now, we are waiting for the approval of the study from the relevant institutional review board.

今後の研究の推進方策 |
We are going to conduct the main experiment (randomized repeated measures in design) with exposure of elderly subjects to whole-body vibration at 15 Hz, 20 Hz and 25 Hz with a peak-to-peak displacement of 2 mm each, and also to a control condition. Measurements of Heart Rate Variability/HRV, leg Skin Blood Flow/SBF, Skin Temperature/ST, Vibrotactile Perceptrion Thresholds/VPT, Thermotactile Perceptrion Thresholds/TPT and functional mobility and dynamic balance tests (Timed Up-and-Go, Parallel Walk, and Single Leg Stance tests) will be carried out before (pre) and after (post) exposure to different experimental conditions. The data collected for various tests employed in this study will be compared to determine the differences between pre- and post-exposures values for each exposure condition and also between the four exposure conditions. In line with the observed responses induced by acute exposure of subjects to whole-body vibration, a study (two-period, two-condition crossover design) with a limited number of subjects will be conducted to evaluate the effectiveness of chronic exposure of WBV in inducing long-term improvements in peripheral circulation and balance among the elderly. A two-period, two-condition crossover design will be adopted where all subjects will experience both intervention and control sessions with an 8-week washout period between interventions. The same variables describe above will be measured in that study. Also, blood samples collected before and after interventions will be analyzed for different relevant plasma biomarkers.

Although the preliminary experiments were conducted in time, there are slight delays in conducting the main experiments due to various essential arrangements and experimental set-up, and also due to delays in obtaining the approval from the institutional review board of Yamaguchi University School of Medicine. Therefore, to conduct the main experiments, incurred amount will be used during the next fiscal year. In the main study, healthy elderly subjects will undergo exposure to different experimental conditions. For this purpose, honorarium for the participants will be necessary. Also, collected blood samples will be analyzed for different relevant plasma biomarkers. We will need to buy analysis kits for each of those biomarkers. For collection of blood, we need to buy items like syringe, glass tubes etc. For measurements and recording of data, we will also need to buy stop watch, sphygmomanometer, paper, toner etc. Also, to input data, an assistant will be necessary. The research outcomes will be reported in relevant scientific conferences. The trip costs for joining such conferences will be required.
