【背景】教育研修病院は、先端医療と人材育成の場として「チーム医療の範」となることが求められる。そのよりどころとして「医療のプロセスを追跡して評価できる」/「開示に堪える」医療記録の記載が基本戦略といえる。しかし、日本で導入されてきた電子カルテシステムは、医療の質と患者安全向上の面で必ずしも成果が得られていない。【研究過程】研究代表者らは、その原因が医療記録の基本構造にあると考えた。臨床実務に就くすべての医療専門職種が参画するワークショップを国内外において年次的に開催して、(1)患者安全の観点から医療記録の構造と記載の現状、(2)救急医療のプロセスに即し情報共有に堪える医療記録、(3)プロトタイピング法によるNarrative dataの取り扱い、(4)データ活用と保護の両立のためのセキュリティ要件の観点から段階的に検討した。【結果】(1)説明責任を担保できる医療記録の構造は、時間軸の通覧性、多角データの概括性が要件であり、担当者がプロセス情報を共有、安全なPDCAの推進を支持する「clinical Navigation System」と称する機能を提案した。(2)その際、救急医療での運用も考慮し、職種横断的な業務フローを設計(3)患者の個別の問題点とそれぞれに対する介入(intervention)、および、それによって生じる患者の容態変化を整理して記述し、検索できるNarrative dataの集積システムのモデルを作成することによって検討した。(4)その際、情報欠落、破壊、重複が回避できる要件を検討、予防的保守と緊急時即応準備(EP)項目をまとめた。【今後の方向】以上を国際医療情報学連合(IMIA)の医療情報の安全に関する部会の中期課題の企画資料として提出し、幹事会の審査をうけて、次期会議〔広島〕の中心課題として採用された。その成果は「広島宣言」に盛り込まれる見通しである。
