膵ポリペプチド細胞の再生における実験的・臨床的研究

研究概要

スンクス膵臓におけるPP細胞は総胆管の右側にある右葉膵臓に制限的に分布している。本年度はKAT系スンクスを用い,右葉膵臓を完全に切除したPPノックアウト実験より,もともとPP細胞の存在していない左葉膵臓にもPPタンパク発現細胞が発現できた。その発現細胞はラ氏島細胞のglucagon陽性細胞或いは/及びinsulin陽性細胞に共同局在であることが確認された。その細胞がBrdU抗体に陽性反応することが認められた。PP細胞の発生はα細胞およびβ細胞とともに,共通な先駆細胞,PYY陽性反応細胞がある。PYY/glucagonおよびPYY/insulin陽性細胞からPPY/glucagon/PPあるいはPYY/insulin/PP陽性細胞に,さらにPP陽性反応細胞に分化する機序がある。本実験におけるPP細胞は膵内分泌系のα細胞及びβ細胞ともに再分布することについて,PP細胞の多態形成に機序と一致していた。
更に,臨床からの膵臓切除標本を用い,膵頭部切除後(膵頭十二指腸切除術,PD)の残膵(膵体尾部)におけるPP細胞の再分布の確認ができた。しかも,再分布できたPP陽性反応細胞の量は術後の経時的な観察にて,時間との正相関であることが確認された。
