研究実績の概要 |
Heart failure (HF) is one of the leading causes of death in the world and the burden of the disease is high among the populations in lower- and middle-income countries. In Mongolia, cardiovascular disease (CVD) is the 3rd and 2nd causes of population and inpatient morbidity and it is estimated that one in third of all deaths are caused by CVD. Evidence has indicated that patients with HF are often unsuccessful at adequately performing self-care which leads to re-hospitalization due to worsening of symptoms. Therefore, e purpose of the study is to examine the effect of a nurse-lead care program for patients’ HF on self-care, quality of life, and re-hospitalization. Furthermore, this study will explore the feasibility of nurse-led intervention program and its potential impact in improving patient outcomes in developing countries. This study will be conducted over 4 phases. The Phase I (from April 2019 to March 2020) was mainly dedicated in review of literature about the nurse-led patient education strategies, heart failure management, and heart failure programs that reduced readmission to the hospital after inpatient treatments.
In August 2020, the researchers and the Mongolian research collaborators, and clinical nurses, who practice in the areas of cardiovascular nursing, met at the Mongolian National University of Medical Science in Mongolia and had a hearing about nursing practice and nursing intervention to the patients with cardiovascular disease. Also, the researchers assessed the system and environmental factors that allow nurses to provide patients education.

理由
Due to the COVID-19 Pandemic, the Country of Mongol had placed the international travel restrictions since February 2020. Also, our research collaborators including the research institution and the hospitals are under quarantine. Due to the prevention of the infection, the use of the proposed hospital site is pending and there is a potential of change of the data collection site. We were supposed to submit institutional Review Board (IRB) approvals to the University of Tsukuba and Mongolian National University of Medical Sciences; however, due to the circumstances of the pandemic, there are still uncertain factors with the study schedule.

今後の研究の推進方策 |
In Phase II, (April 2020 -February 2021) During the time of pandemic related restrictions, we are aiming to proceed with translation of measurement tools and educational materials in Mongolian language with collaboration of Mongolian research collaborators. Once the data collection site is redetermined, we will proceed to IRB approval executions. By the end of the phase II, we hope to do recruitment and training of data collectors and nurses and conduct a pilot study.

We were planning to do an assessment of lifestyle and diet characteristic of Mongolian population in the winter, and to co-develop research protocol with the Mongolian research collaborators; however, due to the travel restrictions, the travel was cancelled.
