In the first year, we used a established rat model in the pre-diabetic stage of OLETF rats, a model of T2DM, with LETO rats as genetic control. We clearly demonstrated that Four months of mild or moderate exercise from pre-diabetes was effective in significant normalization of glycemic parameters with hippocampal molecular normalization through MCT2, glycogen, and BDNF levels with substantial improvement of memory dysfunction in early-T2DM rats. In the 2nd year, from various organ investigations, we found the hippocampus is affected earlier than other organs like the heart, kidney. Here we investigated whether a daily walking intervention could improve the fasting blood sugar level in newly diagnosed diabetes mellitus participants in rural Bangladesh who were not on medication. A rural population of 1,045 people in Bangladesh was screened for diabetes mellitus. One hundred fifty-six people, including 87 diabetes mellitus participants, went through a 5-month daily walking program (twice daily, 1.5 km walking each time). In our initial screening, the prevalence of newly diagnosed diabetes mellitus cases was 8.5% in the rural population. The mean fasting blood sugar level in these new diabetes mellitus cases was 10.0 ± 0.33 mmol/L. After five months of exercise, both the fasting blood sugar and 2h OGTT sugar levels were significantly improved, and no DM (diabetes mellitus) cases were on medication. The current research findings show that a walking program can be a potential lifestyle intervention to combat the diabetes mellitus epidemic in a poor rural context in Bangladesh.

理由
Reason and status: Although COVID-19 is affecting a lot of works, fortunately, the current research is on the right track both from the contexts of the timeline and the number of research activities scheduled to be done (100% done with target milestone achievement) with the proper management of project budget and funding. We did the target works in the first year and also the scheduled works in the 2nd year. In this Covid-19 crisis, we published the human study this year in February. We could also continue animal studies and expect to complete all the works by this year.

今後の研究の推進方策 |
In the 3rd year, we will do more focus on human study. We already published a part of that research. Now we are conducting the survey with different exercise regimens and duration. Also, we include patients from various degrees of diabetes. This year also, we will have a number of mobile health screening camps in rural Bangladesh through screening and assessing diabetes by OGTT. In Bangladesh, in a rural setting, it is challenging to measure HBA1c for diabetes screening. Also, we continue animal studies. Here we intend to use fatty diet-induced mice diabetes model and will conduct the exercise. High fatty diet-induced pre-DM and DM mice will be used. Here we will also focus on VEGF angiogenic signaling cascade and the related apoptosis signaling cascade as a molecular investigation.
