G型肝炎ウイルス二重感染がHTLV-1母子感染に与える影響:後向きコホート研究

研究概要

本研究は、G型肝炎ウイルス(GB virus C:GBV-C/HGV)の二重感染が成人T細胞白血病ウイルス(Human T Lymphotropic Virus type 1:HTLV-1)の母子感染に与える影響を、後向きコホート研究によって評価する研究である。これまでに行われた研究から演繹される仮説として、我々は「GBV-C/HGVの二重感染がHTLV-1母子感染を抑制する可能性がある」との仮説を得た。一方、本研究はHTLV-1感染症を疫学的視点から深く理解することも同時に目指している。
当該年度は、HTLV-1抗体陽性妊婦保存血清のうち、出生児のHTLV-1感染の有無が、これまでに行われたHTLV-1母子感染防止事業より明らかな血清のリストを作成し、リストのなかから、無作為に抽出した血清に対しGBV-C/HGV感染率をRT-PCRを用いて計測し、妊婦集団におけるGBV-C/HGV感染率の推定を行った。HTLV-1陽性妊婦180名の検体を調査したところ、RT-PCRによる感染率は約2%と低率であることがわかった。次いで、得られた感染率より、GBV-C/HGV感染の有無によるHTLV-1母子感染率有意差検定に必要なサンプルサイズの推定を行ったところ、必要なサンプル数は約1000検体であることが明らかとなった。必要サンプル数は大きく、計画の変更が必要であることがわかった。そうした研究デザインの変更を行っている。一方、対象となった妊婦集団における抗体陽性率の経年推移や介入の結果に対する知見をまとめている。また、南西日本におけるHTLV-1の由来についても新たな知見を蓄積している。
