研究実績の概要 |
During the third year, we completed dietary assessments in patients with rheumatoid arthritis (RA) on treatment with targeted therapies using the brief self-administered diet history questionnaire and a detailed fish frequency questionnaire to assess the fish consumption in the previous month. Three hundred and thirty-three patients (218 Japanese, 115 Spanish) returned the questionnaires. Disease activity was evaluated at study entry and demographic, clinical and laboratory information was retrospectively collected. In the Japanese cohort, an analysis was performed in 124 patients with all clinical/laboratory data available. Ninety-six patients (77%) were responders to treatment. All patients had fish consumption in the last month and 82% reported intake of n-3 polyunsaturated fatty acids (PUFAs) rich fish (<1 time/week 33%, ≧1 time/week 49%). A higher estimated daily intake of n-3 PUFAs from fish was found in the responder group compared to the non-responder group [median gr, 1.01 (IQR 0.47-1.44) vs 0.51 (IQR 0.34-1.14), p=0.044]. The analysis of 62 patients from the Spanish cohort, thirty-four (55%) responders to treatment, revealed that all patients had consumption of fish in the last month and 77% had consumed n-3 PUFA rich fish (<1 time/week 31%, ≧1 time/week 46%). No difference in estimated intake of n-3 PUFAs from fish was observed in this cohort. These data suggest that fish consumption may have a positive effect on the response to treatment in Japanese RA patients receiving targeted therapies.

理由
Due to the impact of the COVID-19 pandemic, the inclusion of patients in the retrospective phase of this study was delayed and we were unable to start the prospective study as planned. During this third year, the protocol and all prospective study materials were completed and approved by the ethics committee, and patient recruitment for the prospective phase was launched.
Regarding the prospective study, we have conducted an analysis of the effect of fish intake on the response to targeted therapy in the Japanese and Spanish RA cohorts and prepared and submitted abstracts to National and International Conferences. We are now completing the final analysis and will prepare the manuscript for publication.

今後の研究の推進方策 |
We have secured an extension and in the fourth year we will recruit the patients for the prospective study, perform dietary assessments at the initial and subsequent visit and blood sampling. We will collect demographic; clinical and laboratory data at the two visits and perform the laboratory determinations. All data will be entered into the database and analysed. The results obtained in this research will be presented at national and international conferences and prepared for publication in academic journals.

次年度使用額が生じた理由 |
Due to the delays we have experienced in the completion of the retrospective study, we have not been able to carry out the prospective phase of the study. We plan to use the funds for the development of the prospective study, including dietary analysis for the two visits, laboratory data measurements, data analysis and travel expenses to attend conferences and meetings with the Spanish counterpart.
