Borderline ovarian tumors (BOTs), while distinct in clinical presentation and less aggressive than ovarian carcinomas, present unique challenges in diagnosis and treatment. This study represents the first comprehensive integration of whole-exome sequencing (WES) and RNA sequencing (RNA-seq) to compare the molecular characteristics of BOTs with those of high-grade serous carcinoma (HGSC), endometrioid carcinoma (EC), and clear cell carcinoma (CCC). Our analysis included 44 ovarian tumor samples, highlighting genomic similarities particularly with EC, and uncovering a novel transcriptomic resemblance that suggests deeper biological linkages. Notably, the transcriptomes of BOTs showed remarkable similarity to those of ECs, a novel finding that has not been previously reported. This similarity could reflect shared oncogenic pathways or tumor microenvironments, potentially guiding more tailored therapeutic approaches. The study also reevaluates the molecular classification of ovarian tumors, suggesting that BOTs may be more akin to ECs at a molecular level than previously understood. These findings open new avenues for research into the pathogenesis of BOT and may influence future diagnostic and treatment strategies.
