研究概要

第1に、皮表の黄色ブドウ球菌(黄ブ菌)により刺激・活性化された表皮角化細胞(KC)から遊離されるとされる免疫活性物質のIL-18を測定するために、tape-stripping法による角層NGFの非侵襲的な測定方法に順じて角層IL-18の測定法を確立し、そのバイオマーカーとしての意義について検討した。その結果、AD患者の血清IL-18値が健常人より優位に高いのに一致して、角層IL-18値もAD患者の無疹部の方が健常人の無疹部より有意に高値であり、AD患者では皮疹スコアに相関して有意に上昇し、治療に伴う皮膚症状の改善に一致して有意に低下した。また皮疹部の角層IL-18値は血清IL-18値やADの病状指標として広く使用されているSCORADと有意に相関するだけでなく、末梢血好酸球数や血清中のIgE値、LDH値、TARC(thymus and activation-regulated chemokine)値など各種指標と有意に相関した。ADの症状指数として知られるSCORAD、血清TARC値、末梢好酸球数、血清総IgE値などの通常の臨床検査値の中で、特に、ADの重症度や再燃・悪化をよく反映するとされる血清TARC値により顕著な相関が認められた。さらにこの角層IL-18値は皮表黄ブ菌叢の定着有り群が無し群より有意に高く、血清総IgE値から便宜的に高値AD群と低値・中等値AD群の2群に分けて検討すると、血清総IgEの低値中等値AD群でより有意な関連が認められた。従って、角層IL-18値は恐らく皮表黄ブ菌叢の定着に関連して活性化された表皮角化細胞(KC)により産生遊離されて上昇し、血清総IgE値が相対的に高値なextrinsic AD患者群よりは血清総IgE値が相対的に低値なintrinsic ADに該当すると思われる患者においてsuper Th1細胞の活性化を介したADの発症・悪化に重要な役割を果たしていると考えられる。第2に、痒みの強い尋常乾癬病変においてもSemaphorin 3A(Sema3A)と神経線維の表皮内伸長を誘導するnerve growth factor(NGF)が果たす役割を検討し、神経線維の分枝・伸張とNGFの発現が相関関係にあるのに対してSema3Aの発現は逆相関の関係にあることを免疫染色とPCRによるその遺伝子発現の解析により明らかにした。また鼻水を伴う痒いアレルギー性鼻炎の粘膜下層でNGFが発現が増加して鼻腔洗浄液中でNGFの量が増加しNGFの産生に好酸球が関与することが報告されていることから、卵白由来のオボアルブミン(OVA)をアラム加腹腔内注射して感作したBALB/cマウスに鼻腔内投与して実験的アレルギー性鼻炎を作製して、鼻粘膜上皮におけるSema3Aの発現低下とNGFの発現増強を免疫組織学的染色とPCRで明らかにし、この鼻腔内にSema3Aを点鼻投与することにより、対照の生食投与群に比べてマウスの鼻引掻き行動や鼻分泌中の好酸球数を減少させ、組織学的にも鼻粘膜の肥厚や粘膜上皮内の神経線維の数を減少させることを明らかにした。従って、Sema3Aは、痒みの強いアレルギー性鼻炎に対しても新規治療薬となることが期待される。第3に、これまでの痒みを誘発するドライスキンの治療は保湿のスキンケアと抗炎症のステロイドや免疫抑制薬の外用が基本であったが、コラーゲン由来のトリペプチド(CTP)は真皮内の繊維芽細胞に働いてヒアルロン酸(HA)の産生を促すことから、実験的ドライスキンに対して大量のCTPを経口投与することで、皮膚のHA含量を高めて亢進したTEWLを抑え、皮膚のNGFの低下とSema3Aの増加を伴って掻破行動が有意に抑えられることを明らかにした。新しい視点に基づいた痒みのあるドライスキンに対する新規治療法であり、今後の臨床応用が期待される。
