軽度認知機能低下者における認知・身体機能向上のための運動介入法の開発

研究概要

本研究の目的は、軽度認知機能障害（Mild Cognitive Impairment; MCI）及び軽度のアルツハイマー病（Alzheimer disease; AD）患者における認知機能の維持・改善に運動介入が有用であるかを検証することである。40名のMCI者および軽度AD者を無作為に運動介入群とコントロール群に分類し、運動介入群には月に1回の教室型運動介入と歩数計を用いた日々の運動指導を6ヶ月間実施した。アウトカムの指標としては、脳の構造MRI画像、種々の認知機能、運動機能、それに身体活動量とした。
結果：身体活動量は有意な交互作用を認め（P<0.05）、運動介入群で約4000歩から5500歩に増加したのに対してコントロール群では不変であった。運動機能では有意な交互作用を認めた項目はなかった。認知機能では論理的記憶(logical memory)の項目において運動介入群で改善傾向が認められた（P=0.06）。脳の構造MRI画像では、分析方法によってやや結果が異なるものの、一致した結果として本介入によって脳容量が増加するという結果には至っていない。
これらの結果より、本運動介入（月1回の教室型運動介入と日々の有酸素運動）では、MCIおよびAD者の病状の進行抑制に有用とは言い切れない結果となった。今後は、運動量の増加や運動の種類を変化させることによる効果の検証や、運動介入が有用となるような集団の検証などが必要である。
