核酸分解酵素に着目した凍瘡状狼瘡の病態解明と新規治療法の開発

研究概要

凍瘡状狼瘡（chilblain lupus: CL）とは、DLEの1亜型である。指趾・足外縁、耳朶等に、冬期に増悪する高度の角化性紅斑で、中央が陥凹して厚い鱗屑を有し、ときに潰瘍化する。患者は病変部の疼痛を訴える。成人発症であるが、幼少時に凍瘡をきたしやすい等の既往がある患者が散見される。病因は家族性の一部以外は不明である。
家族性凍瘡状狼瘡（ｆamilial chilblain lupus: FCL）は2006年に提唱された疾患である。常染色体優性遺伝性疾患(ヘテロ変異の疾患)である。CLと類似の症状を来す。2007年と2011年に病因遺伝子として、TREX1とSAMHD1がそれぞれ同定された。現在までに、TREX1
の遺伝子変異による本症は2家系、SAMHD1の遺伝子変異による本症は1家系の報告があった。アジアにおいて本症の報告はなかった。TREX1やSAMHD1のホモ変異によりAicardi Gutieres症候群という小児脳変性疾患を発症する。さらに、SLE患者の2％はTREX1ヘテロ変異を有しており、TREX1はSLEの疾患感受性遺伝子でもある。我々は、日本人のFCLの患者において初めてTREX1とSAMHD1の遺伝子検索を終え、TREX1 c.394C>G (p.Pro132Ala)というヘテロの新規ミスセンス変異を発見た。in vitroの一本鎖DNA分解実験において、この変異によりTREX1の機能変異をきたすことを明らかにした。さらに、文献的検索と本症例の結果から、TREX1のDNA分解に重要なアミノ酸（DDEDhモチーフ）あるいはその近傍のアミノ酸の変異をヘテロで有する場合全例で幼少発症CLをきたしているという遺伝子変異臨床型相関を初めて見出し、報告した。
