高齢進行肺癌患者における、VES-13ならびにMMSEの有用性に関する検討

研究実績の概要

本研究では、高齢者の代表的ながん種である非小細胞肺癌を対象に、脆弱性評価指標Yulnerable Elders Survey-13(VES-13)ならびに認知機能評価指標Mini-Mental State Examination（MMSE）について、両者の関連の程度ならびに予後や治療アウトカムとの関連について検討する。
現在までに14施設から93例が登録され、VES-13・MMSE評価を実施した。対象者の年齢は中央値72歳（範囲65-84）、VES-13スコアは中央値1点（範囲0-7）、MMSEスコアは中央値27点（範囲23-30）、また合併症スコアであるCharlson Comorbidity Index（CCI）は中央値0点（範囲0-4）であった。年齢との相関係数はVES-13で0.0002、MMSEで-0.01であり、年齢との関連はみられなかった。またVES-13とMMSEの相関係数は-0.08で、脆弱性が高いほど認知機能が低下する傾向であるものの関連性は弱いことから、両者は年齢と独立して予後・治療アウトカムに影響を及ぼす可能性があることが引き続き示唆された。
ベースラインデータならびに予後・治療アウトカムについては、参加施設からWeb入力するシステムを運用中で、残り一年間で、予定症例数である120例まで症例登録ならびに情報収集を行う。
関連する研究として、70歳以上の進行非小細胞肺癌患者に対するプラチナ製剤を含む併用化学療法の忍容性予測に関する後ろ向き研究を実施しており、198例のデータにおいて、血清アルブミン低値が、腫瘍増大がみられないにも関わらず2サイクルの化学療法を完遂できない症例の予測に役立つ可能性があるとの知見が得られ、論文を投稿し現在revise中である。

理由

現在までに14施設において倫理審査承認が得られ、登録可能となっている。専任のインタビュー担当者を雇用して各施設へ適宜派遣しており、参加施設医師の負担を軽減しつつ、高齢者機能評価に関する判断基準を揃えて評価するという重要な点を考慮した体制を維持している。
参加施設からの症例登録ペースが当初予測よりも遅いことから、参加施設で対象症例が発生した場合に確実に組み入れられるようチェックを強化する必要があると考える。

今後の研究の推進方策

28年度は本研究の症例登録を続行し、目標症例数120例へ向けてデータ収集に努める。症例集積を促進するために、試験開始時に定めた2レジメン以外の化学療法を受ける患者についても本研究への組み入れを継続する。
参加施設のうち病床数の多い数施設については、研究代表者自身が直接出向いて適格症例の有無を定期的にチェックする。
関連する研究である「70歳以上の進行非小細胞肺癌患者に対するプラチナ製剤を含む併用化学療法の忍容性予測に関する後ろ向き研究」については、現在論文投稿中で、revise後の再投稿を済ませておりその結果待ちである。

次年度使用額が生じた理由

症例登録のあった施設が名古屋市内でインタビュー担当者の交通費が発生しなかったり、施設内スタッフで高齢者機能評価を行ったためインタビュー担当者の派遣が不要であったケースがみられたため。

次年度使用額の使用計画

今後登録される症例について、インタビュー担当者の費用やデータ管理費用などで使用する。
