研究概要

平成元年度は腎移植片に対して免疫寛容を導入する目的で組織不適合間の雌雄の異なるbeagle犬を用いて術前全リンパ組織照射(FLI)、腎移植時の腎donorからの骨髄移植(BMT)、さらに術後にFK506の小量投与の3者併用法を用いて検索した。(1)FLIは150rad/日、10日間計1500rad、BMTはPHA無反応BMC、平均2×10^7/kg、FKは0.08mg/kg/日90日間投与法である。(2)この3者併用(I群)により骨髄donorからの腎移植片(KG)はFK中止後も生着し続ける症例があった(3/6例)。一方BMTを併用しないFLIをFKの併用(II群)では腎は生着延長するがFKを中止すると腎は約2〜20週の間に全例拒絶された。(3)I群でFK中止後約30日後に2次的に皮膚移植を行ったところ、2/3例において骨髄・腎donorからのSGは約2週間以内に拒絶された。またII群ではFK中止後、腎は拒絶されるが同様に移植した腎donorからのSGは2/4例が生着し続けた。(4)I、II群のSGについて約200日後にSG内のIa陽性細胞の有無およびその由来について抗Iamonoclonal抗体(ISCR3)、lymphocytotoxic抗体(Host抗Donor)、(Donor抗Host)を用いてdouble labelling Immunofluorescenceを行った。その結果I群のSGに存在するIa陽性細胞はdonor由来であり、一方II群のSGのIa陽性細胞はrecipient由来の細胞で置換されていることが判明した。この所見はtolerantSGを再び各々のdonorへ戻し移植を行うとI群では全て生着し、II群では全て拒絶されることから確認できた。(5)I群のrecipientの未血や骨髄の性染色体分析によるchimeraの検索では陰性であったが、microchimerism成立の可能性は否定できない。(6)リンパ球混合培養抑制試験ではtolerantのrecipientの血清中に術前のrecipientのリンパ球に対するdonorの反応をのみ抑制する抗体が出現し持続する傾向がみられた。(7)本法はazathioprineやcyeloaporinとの併用でみられた骨髄抑制や肝障害などの副作用がほとんどなく、安全な方法で臨床応用に向けてさらに検討中である。
