移植臓器拒絶反応の免疫学的制御

研究概要

(1)ヒト腎移植ではcyclosporine(CYA)とPrednisdone(RPD)投与法により安定した成績を得ているが、さらにdonor特異輸血(DST)を加えると生着が高まる(田口、阪上、深尾)(2)DSTによる前感作はazathioprine(AZA)を間〓的に併用することにより十分防止できる。DST患者の脾細胞とマウスミエロ-マ細胞とhybridomaを作り、脾細胞中に抗原特異的抗イデイオタイプ(Id)抗体を産生するクロ-ンを得た。あるクロ-ンはrecipientのT細胞と向う抗Id杭体を産生した(阪上)。(3)DST症例(CYA,PRD症例)の拒絶反応で細胞浸潤型と血管病変の混合型にはOKT3が有効(田口)。(4)CYA併用DST症例では末梢リンパ球のサブセットにあまり影響を与えない。OKT3やステロイドの大量の与のmonitoringに有効(深尾)。死体腎移植の拒絶(-)例で移植早期にLeu3^+8^+,Leu4^+IL2R^+が減少、以降前値より低値の維持。一方拒絶(+)例の早期ではそれぞれ同様の傾向だが、その後高値が持続する(阪上)。(5)イヌの同種皮膚や腎移植で術前recipientのリンパ組織への選択的小量X線勝者、臓器移植時のdonor骨髄細胞の投与が移植片生着に有効で術後FK506を中止しても腎は生着し続け、さらに腎donorからの皮膚移植片(SG)が生着し続けるが、第3着からのSGが拒絶されるという免疫寛容が導入できた。生着したSGにはdonor型のIq陽性ランゲルハンス細胞は維持されていることが確かめられた(渡部、柏木)。(6)アロ杭原に対するdrungーinduced toleranceのin vitroにおける誘導結果から、組織不適合間のB6C3F1-C3Hを照射後、同系の骨髄細胞移植と同時にB6C3F1の脾細胞を移入するとB6のSGが免疫寛容を各徳する。この寛容の維持には一定期間のキメラの状態は必要だが、長期維持は必ずしも必要としないことを示した(姫野)。
