研究概要

1.左足底に原発性結節性黒色腫をもつ57才男性につき即発γ線法による診断後、京大炉で治療照射。腫瘍は一時的な抑制を経て再増殖に転じた。切除手術施行,病状観察中。2.諸種濃度のD,Lー又はLー ^<10>BーBPA液で黒色腫細胞を培養,細胞内 ^<10>BをICPーMS測定。1例:BPA200μg/mlで21.4と12.6μg/g(即ち1.7×10^8)細胞。L/DLは常に1.5〜2.0。3. ^6LiF含有の中性子吸収体とポリエチレン製中性子反射体を組合せて作ったコリメ-タの照射線量制御能力をファントム実験で確認。4.武工大炉照射場の構造をリチウムタイルを用いて改善する計画は炉本体のトラブル発生による運転停止のため施工延期。5.本療法では当初使用の ^<10>BーBPA・Hclに代えてBPA果糖錯体が現用されている。後者の場合,全身麻酔よりもジアゼパムによる鎮静が優ることを確認。特に高令者に適切。6.黒色腫患者9例の皮膚反応で推定した正常皮膚の最大安全フルエンスは,皮内 ^<10>B濃度3〜5μg/g,照射野面積50cm^2以下なら1.0〜1.1×10^<13>n/cm^2で,照射野が大きいほど減少。7.腫瘍加温は辺縁部の局所血流増加(1.6倍)を越すが,中心部も含んだ腫瘍全体の血流増加は認め得ない。腫瘍/皮膚の ^<10>B濃度比の上昇は意外に少く1.2倍。8.(1)試料量100mg,高温加熱,遠心分離機使用により2hr以下で分析可能化。(2)皮下脂肪の分解には現行の(H_2O_2+HclO_4)液より硝酸が有利。(3)同位体希釈法は予備実験で有望。9.酵素法で光学純度ほぼ100%のL体とD体を数gブフ採取。溶解度法は時間を要し,純度・収量ともに劣る。HPLCは純度検定に好適。10.ヒト黒色腫MRIは複雑で,文献にみるTi・T_2短縮は必ずしも起らぬが,腫瘍進展域が正確につかめ治効判定に有用。ハムスタ-黒色腫のviable部と壊死部の鑑別可能。11.皮膚黒色腫24,頭頚部黒色腫22症例に速中性子を照射,前者19,後者10例で局所制御を達成(手術併用例を含む)。速中性子療法は熱中性子捕促療法に比し皮膚損傷大。
