| Outline of Annual Research Achievements |
ヒト脳発振現象の実データ記録とその解釈で以下の結果を得た。
１）大脳皮質領野レベルでは、成人てんかん患者の皮質脳波で、発作時DC電位が、てんかん焦点において高周波振動(HFO)に先行し、新皮質てんかんでこの傾向が強いこと(Daifu et al,2016)、発作間欠期においても緩電位とHFOの相互作用が、てんかん焦点で観察され(Daifu et al., 2015) 、グリアのてんかん病態への関与を示した。この結果に基づき理論的モデルの構築検討を開始した。
２）非侵襲的な脳磁図で、ワーキングメモリ課題中の頭頂後頭部のα律動が、直接の情報処理よりは、視空間的入力からの注意の解放によって変容することを示した(Ichihara-Takeda, et al., 2015)。
３）皮質領域間の振動現象の探索に、皮質単発電気刺激を用いて外的入力が皮質の振動現象に及ぼす影響を検討した。皮質・皮質間と視床・皮質間の入力経路の比較検討から、入力経路・入力層によって皮質に生成される周波数帯域が異なることを明らかにした(Kobayashi et al., 2015)。また、固有振動を持つ皮質では、外的入力が位相同期を惹起する可能性が示された。誘発される高周波数の振動現象はノンレム睡眠時には増強、抑制、再度増強に転じる一過性の動態を示し、再増強は前頭葉で顕著であった(Usami, et al., 2015)。皮質皮質間誘発電位を皮質間結合性の指標として、読字課題下には読字ネットワークの皮質間結合が変容することを明らかにした。
４）時相的観点から、ヒト脳の発達段階における小児期てんかん性脳症の頭皮脳波を解析し、大量の病的HFOが非侵襲的に検出されることを示し、病的HFOが、てんかん性脳症における認知障害の発生に関わることが示唆された(Kobayashi et al., 2016)。
５）臨床応用されているが未だその機序が不明である迷走神経刺激について、外的な律動制御の作動原理に脳電位の陽性変位が関連することを明らかにした (Borgill et al,2015)。

Current Status of Research Progress
1: Research has progressed more than it was originally planned.
Reason
1) てんかん病態において低周波数と高周波数の振動現象が密接に関わることを明らかにし、発作発現に至る病的遷移の解明に向けて解析が進んだ。生理的な振動現象について、異周波数間結合の解析を用いた皮質領域内の低周波数と高周波数間におけるクロストークの覚醒・睡眠段階に応じた動態、随意運動時の低周波数と高周波数の活動の時間空間的な関連について解析を開始した。皮質領域間の脳律動現象に関しては、単発刺激を外的入力のモデルとすることで、ヒト大脳皮質に誘発される振動動態を明らかにし、さらに睡眠・覚醒の変化および脳機能発現時のネットワークレベルの変容を解明した。局所神経回路レベルでは、正常およびてんかんモデルラットにおけるミクロ及びマクロ脳波記録の準備が概ね完了した。
２）これら病的および生理的振動現象の異周波数間・領域間の情報伝達の方向性の解明、併せて理論的数理モデルの構築を目的してB班との密な連携をとり、実際の解析にむけての準備が概ね完了した。
３）時相的な観点から、発達段階における小児期てんかん患者において、病的HFOが頭皮記録から観察可能であり、てんかん性脳症としててんかん性異常脳活動が認知・行動発達障害を阻害する病態と密接に関わることを明らかにした。
４）内的・外的律動制御の作動原理の解明に関しては、本年度は迷走神経刺激を用いた外的な律動制御の作動原理に脳電位の陽性変位が関連することを明らかにした。

| Strategy for Future Research Activity |
１）大脳皮質領野レベルでは、成人てんかん患者の皮質脳波の広域周波数帯域解析（含、異周波数間結合）を継続し、低周波数と高周波数の振動現象の出現様式・相互作用の観点から、覚醒から睡眠、そして、発作間欠期から発作への遷移動態を明らかにし、事象発現時の振動現象を計測・解析する。
２）皮質領域間では、侵襲的計測（皮質皮質間誘発電位、皮質領域間の周波数相関解析）、非侵襲的計測（脳磁図）から、正常機能ネットワーク、てんかんネットワークにおける振動現象の発現および遷移機構を推進する。これら病的および生理的振動現象の異周波数間・領域間の情報伝達の方向性を明らかにし、併せて数理モデルの構築をめざす。B班（数理モデル解析）との密な連携を継続し、正常・てんかん病態関連の皮質脳波データを選定し、数理解析に着手する。
３）局所神経回路レベルでは、正常およびてんかんモデルラットにおいて、広域周波数帯域で局所電場電位と活動電位の記録を開始し、これらの相互作用、刺激による外乱に伴う振動現象の変化を探る。てんかん患者を対象としたミクロ記録の準備を進める。
４）ヒト脳の発達段階に特異的なてんかん性脳症患者を対象に、HFOと低周波を包含する広域周波数帯域脳波解析を行い、てんかん性脳症における認知障害発生の過程を解明する。病変部位、発達段階や脳機能障害の状態との対応関係を分析することで、神経ネットワークを病的HFOが阻害するプロセスを明らかにする。併せて病的HFO発生機構の解明を進めるために、イオンチャネルに関わる遺伝子変異モデル動物ならびに神経ネットワークのシミュレーションを用いた研究の準備に着手する。
５）C班（介入研究）と連携し、外的(迷走・三叉神経刺激、皮質の直接電気刺激)な律動制御の手法を用いて、局所およびネットワークレベルの生理的および病的振動の変容について、発作頻度の変化、記憶など機能変化と併せて解析を行う。
