| Research Abstract |
シスプラチンとメチルプレドニゾロンを併用投与し、どのような投与方法によりシスプラチン腎毒性が予防され得るかを、腎機能的に検討した。ラットを使用した実験系で、メチルプレドニゾロン100mg/kgとシスプラチン6.5mg/kgを同時に投与しても、腎毒性は予防されないが、メチルプレドニゾロンを1日前、4時間前、2時間前に投与し、シスプラチンを投与した結果では、1日前に投与しても効果は認めないが、4時間前、2時間前に投与した群では、血清クレアチニンおよびBUNの上昇は有意に抑制された。この結果は、メチルプレドニゾロンを数時間前に前投与することにより、シスプラチンの腎毒性発現を予防することを示している。組織学的検討では、尿細管細胞の障害される範囲に差を認め、メチルプレドニゾロンの前投与群では、障害される細胞の減少が観察された。電子顕微鏡的観察においては、個々の尿細管細胞内小器官に特徴的な所見は認められなかった。
メチルプレドニゾロンの予防効果の機序を解明するために、自立豚腎尿細管細胞を使用して、in vitroにて実験を進めたが、メチルプレドニゾロンの直接効果は認められず、この結果により、メチルプレドニゾロンが直接的に腎尿細管細胞のシスプラチンに対する抵抗性を高めているのではないことが判明した。
今後は、in vivoでのシスプラチンの動態を両群間で比較検討し、メチルプレドニゾロンの予防効果の機序を解明していく予定である。
