膵切除残膵の再生促進と幽門輪温存術式の改良

Research Abstract	広範膵切除や拡大郭清後には程度の差こそあれ消化吸収障害や耐糖能低下が免れえず、その場合には全胃幽門温存下になされる膵頭十二指腸切除により切除残膵の機能的、器質的再生を促すための工夫と創意は是非必要である。膵癌はリンパ節のみならず神経叢にもしばしば転移する特性をもっている。しかし上腸間膜動脈周囲神経叢を全周性に郭清すると、高度の消化吸収障害と共に、膵の萎縮と膵機能低下の悪化が惹起されることが知られている。従って腸間膜根部の完全郭清は残膵の再生能を阻害し、ひいてはその残膵機能にも悪影響を及ぼすので、本郭清に際しては格別の工夫が必要である。そこで腸間膜根部横断標本を作成し(14)リンパ節と上腸間膜神経叢との相互位置関係を解析したところ、上腸間膜動脈の外膜を直接包む神経層の3mm幅以内には殆どリンパ節は存在せず、それより外方には術中識別不能の小リンパ節を含む(14)転移リンパ節が両側性に散在することを実証した。従って本動脈右半周領域は神経叢もろともリンパ節を郭清し、左半周は外膜より3mm幅温存下にその外方のリンパ節を含む組織を郭清し、照射を追加する方法がバランス良い術式であるとの考えに至った。また同部へのリンパ行性癌転移経路と傍神経性癌転移経路とは異なることを、今回あらたに購入したマイクロ顕微鏡を駆使することにより証明した。
