Research Abstract |
We analyzed the cortical distribution of threshold currents necessary for evoking electromyographic (EMG) responses. Current strength increased as a function of the square of the distance from the best point with the lowest threshold. Therefore, the best point could be mathematically determined by the threshold current intensities of at least 6 cortical points. This is because the currents distribution can be fitted by parabola or z=AX^2+BXY+CY^2+DX+EY+F.Based on this principle, we developed intraoperative functional mapping system for determining the central sulcus, motor cortex and sensory cortex using the cortical distribution of threshold currents for evoking EMG responses. The system consists of the stimulating, recording, analyzing, and displaying systems. These systems are controlled by a computer. The computer program for controlling these systems was also developed. After developing this system, we used this system in the operating room. We confirmed its practical and clinical usefulness. The best point with the lowest threshold could be easily determined by this system. The location of the theoretically determined best point was identical with that of the motor cortex or precental gyrus determined by radiological images.
