Research Abstract |
Tumor Necrosis Factor (TNF-alpha) or Cachectin has been implcated as a mediator of many of the adverse host responses to injury and infection. TNF has been shown to be elevated in the serum of burn patients during the acute phase of burn injury. Also, the level of these elevations contributes to delayd wound healing in such patients. In this study we evaluated the serum TNF-alpha levels of burn patients and the patients of keloid and hypertrophic scar to ascertain whether elevated levels of this mediator persists in these patients, leading to compromised wound healing. Our results indicate that TNF-alpha is significantly elevated in patients with major burns, up to 5 years after the injury. This may pose a contributing factor towards the increased rate of infections and delayd wound healing seen in these patients. We are presently, prospectively looking at TNF levels and scar formation, as there seems to be a yin-yang relationship between TNF and scarring. Studies suggest that TNF in low concentrations helps wound healing, whereas higher concentrations may be detrimental.
