消化管内分泌と消化管運動を連動させる新しい腸管神経叢

Research Abstract

消化管の平滑筋層は筋原性の自動能をもつ。これは筋層内にあるペースメーカー細胞が発生する膜電位変動(緩徐波)が一般の平滑筋細胞に伝搬し、その結果平滑筋細胞が同調して律動的に収縮するものである。さらにこの律動収縮を神経がコントロールすることで消化管運動が起こると考えられている。
本研究は以下の点を明らかにすることを目的とした。1)ペースメーカー細胞を形態学的に同定する。2)平滑筋層の律動運動を修飾する壁内神経を解析する。3)内分泌細胞を含む粘膜上皮側からの刺激に対する消化管運動の応答のメカニズムを探る。
これに関連して次の成果が得られた。
1)イヌ、モルモット、マウスの結腸の内輪走筋層粘膜側表層にはペースメーカー細胞が存在する。そこには形態学的に特殊な平滑筋細胞が認められる。特にモルモット結腸の逆蠕動発生部位からは規則的な律動運動が計測され、そこにはより多くの特殊平滑筋が分布している。
2)ペースメーカー細胞と考えられる特殊平滑筋は癌原遺伝子c-kitの産物c-Kitレセプターを発現している。ペースメーカー細胞はこのシグナル系を介して増殖分化する。
3)壁内神経叢については、ここに神経特異的カルシウム結合蛋白ニューロカルシン陽性神経が分布する。神経節ニューロンとその終末に免疫活性があるほか迷走神経由来の知覚神経終末にも反応が認めらる。
4)結腸の粘膜上皮には癌抑制遺伝子APCが発現し、その遺伝子産物はβ-カテニンやhDLGと共存している。結腸上皮細胞の増殖や分化に関するシグナル伝達にAPC遺伝子産物は重要な役割を果たしていると考えられる。
今後の課題として、粘膜刺激が平滑筋の自動能にどのように関与するか、また壁内神経を介して消化管運動に与える影響についてさらに詳しく解析する必要がある。
本研究は小林繁を研究代表者として平成7年度に開始されたが、平成9年度に鳥橋茂子が引き継いでまとめた。
