肺癌におけるperipheral airway marker発現とその臨床的意義

Research Abstract |
正常肺では、種々の特異蛋白質が存在し、肺サーファクタント関連蛋白質であるSP-A,SP-B,SP-C,SP-Dおよび細気管支無線毛上皮細胞(Clara細胞)由来10kilodalton蛋白(CC10)が知られている。本研究では、肺癌におけるSP-A,SP-DおよびCC10の発現を免疫組織学的に検討した。SP-AとCC10は肺腺癌のみに発現が認められ、陽性率はそれぞれ56%と32%であった。また、SP-Dは肺腺癌の54%、肺扁平上皮癌の30%において陽性であった。癌性胸水におけるSP-A,SP-DおよびCC10定量では、肺腺癌由来の癌性胸水においてSP-Aが47%、SP-Dが45%で高値となったが、CC10に関しては高値例を認めなかった。SP-Dは肺扁平上皮癌由来の癌性胸水においても33%で高値であった。病期I期の肺腺癌手術例において、間質誘導の程度と術後再発について、CC10の発現との関連を検討した。CC10発現陽性腺癌例では術後再発が13%に対し、CC10発現陰性腺癌例では43%であった。また、腫瘍内の間質誘導についても、CC10発現陰性腺癌例では陽性腺癌例に比較し有意に間質誘導の程度が高度であった。以上の結果から、SP-AとCC10は肺腺癌に特異的な免疫組織学的マーカーであるが、SP-Dは肺腺癌と肺扁平上皮癌に発現している可能性が高いと考えられた。さらに、CC10はphospholipaseA2およびphospholipaseCの活性を阻害することが報告されており、肺腺癌におけるCC10の発現は予後良好の指標となる可能性がある。
