神経自家移植による直腸癌手術後自律神経機能再現に関する基礎的研究

Research Abstract

大動物(犬)を使い、擬似的な直腸癌手術(直腸切除と神経移植)を行い神経移植を伴う直腸癌手術に対する手技の習熟と術後の自律神経機能(排尿)の回復過程の観察(免疫神経組織化学的手法と電気生理学的手法)を行い、移植神経の生着と機能を検証した。
(1)骨盤内臓神経の一部を切断、切除し、同部位に腓腹神経を置換自家移植した神経移植群を作成。移植された神経の生着と神経路の再生を確認する目的で、膀胱壁より神経細胞追跡標識物質を取り込ませ移植された神経繊維を通って逆行性に脊髄の神経細胞を標識する時期を検討した結果、移植後5週目で仙髄に少数の標識細胞が認められたが、その数は不十分であった。
(2)正常時の排尿機能が移植された神経によってどの程度温存されるか正常、神経切除非移植群、神経移植群の3者において膀胱内圧を電気生理学的(cystometrix investigation)に比較検討した結果、移植後第10週目では全く膀胱機能の回復は認められなかった。20週目での検討で、わずかながら回復が認められたが、膀胱機能としては不十分であった。
小動物ラットを使用した実験成績からは、術後の排尿機能回復に神経置換自家移植が極めて有用である可能性が十分示唆できた。しかし、大動物による検討では不十分な評価しか得られなかった。したがって大動物を使用しての再検討が不可欠であると考えられた。
