内視鏡生検材料による消化器癌の術前分子生物学的悪性度評価法の研究

Research Abstract	胃癌および大腸癌の内視鏡下生検標本および手術標本から腫瘍組織を採取後直ちにGuanidinum Thyocianateを含むRNA抽出液に入れ、可及的早期にFenol- ChroloformにてRNA分画を得た。増殖関連因子(CyclinD1,CyclinE),浸潤関連因子(urokinase-type plasminogen activator:u-PA,72KD type IV collagenase:MMP2),血管新生因子(vascular endothelial growth factor:VEGF,platelet-derived growth factor A:PDGF-A),免疫抑制因子(IL-10,Tumor growth factor-β:TGF-β),癌関連因子(Melanoma Antigen-encoding Gene-1:MAGE-1,c-Met,C-erb-B2)を癌の生物学的悪性度因子として選択した. 現在胃癌においては手術時摘出標本胃癌組織150例,内視鏡生検胃癌組織180例を集積し,うち約半数においてはmRNAの抽出を完了し、種々の生物学的悪性度因子mRNA発現(RT-PCR法)、病理学的悪性度因子および予後との相関を解析し,有為差決定の結果, 1.早期癌においてPDGF-Aがリンパ節転移の有無と相関し、早期胃癌の術前悪性度評価因子の一つになる可能性が示された. 2.また、進行癌においては、 (2)TGF-βがリンパ節転移の予測因子となる可能性(3)PDGF-AおよびTGF-βが生存期間と強い相関を示し、独立した予後因子である可能性が示された.
