Research Abstract

パーキンソン病患者において、床に引いた横線を見せると「すくみ足現象」が直ちに改善することから、視覚刺激の方向性が歩行時の感覚運動連関に重要な意味を持つことは明らかである。本年度は、パーキンソン病患者における血流変化と健常者における脳磁場変化を、視覚刺激の方向性の観点から検討した。
1. 軽症から中等症のパーキンソン病患者10名と健常高齢者10名を対象に、毎分13mの速度で動くトレッドミル上での歩行時の局所脳血流変化を記録した。進行方向に対して横方向に等間隔(30cm)でひかれた線で引かれた線をまたいで歩く条件(横線条件)と、進行方向に対して平行にひかれた2本の線の間を歩く条件(縦線条件)を、トレッドミル上で5分間行ない、課題開始30秒後に99mTc-HMPAOを経静脈投与し、有意な局所脳血流を検出した。パーキンソン病患者群は、横線条件において歩行の改善を認め、左小脳半球、両側後部頭頂皮質、右外側運動前野、右前帯状皮質に血流増加が認められた。パーキンソン病で機能の保たれている小脳、外側運動前野が、歩行運動改善に役割を果たすことが示された。
2. CRT画面上に提示された輝度の正弦波変調した縞模様が、静止状態より縞と垂直方向に移動開始した際に右手示指で反応する課題を健常成人4名に課した。縦縞が横方向もしくは横縞が縦方向に動く場合の2通りの方向と2種類の運動速度の計4通りの課題において、脳磁場反応、反応時間を記録した。反応時間は、速い運動の場合は240-319ms、遅い運動の場合は269-383msであり、後頭部脳磁場反応の頂点潜時はそれぞれ183-205ms、189-213msであって、視覚刺激の移動方向とは無関係であった。健常人においては、視覚移動認知の段階では視覚刺激の方向性は影響しないと考えられる。
外側運動野、小脳を含む視覚運動連関が、歩行運動における重要な役割を示すと考えられる。
