Research Abstract |
We have investigated the extracorporeal photopheresis for the treatment of rejection in small bowel transplantation. We evaluated the efficacy of the photopheresis using the rat small bowel transplantation model in the 2000th year. Based on this finding, we proceeded the research using the canine model in the 2001st year. After the operation of small bowel transplantation, the leukocyte apheresis system was accessed the peripheral vein of the canine and leukocytes were collected as buffy-coat by the centrifugal separator of this system. 0.4 μg/ml of methoxsalen were added in this buffy-coat and the buffy-coat then passed through a 100 μm-thick-cassette, where it underwent a 180-minute exposure to ultraviolet A light, yielding an average exposure of 2 J per square centimeter of leukocyte-surface area. The schedule of photopheresis were days 1 and 2, 5 and 6, 10 and 11, 15 and 16, and 20 and 21 during month 1 after transplantation, for a total of lO photopheresis procedure per canine. The condition was checked clinically and histopathologically. Rejection episodes were observed in 17 % of photopheresis group clinically and 66 % pathologically (100 % each in control group). Moreover, the pathological degree of rejection in photopheresis group was slighter than in control group. The extracorporeal Photopheresis significantly decreased the risk of rejection after small bowel transplantation.
