最適な糖尿病スクリーニング法の開発とその県レベルコホートにおける実証

Research Abstract

糖尿病の有病者数は著しく増加し続けており、日本人において最適な糖尿病スクリーニング方法を確立することは極めて重要な課題である。申請者は「虎の門病院健康管理センター臨床疫学研究プロジェクト(TOPICS)」における人間ドック受診者の既存のデータベースを活用し、糖尿病発症の新たな予知因子や生活習慣を解明することを目的として大規模疫学的臨床研究を実施した。その結果、申請者は、軽度の肺機能低下が既知の糖尿病発症の危険因子とは独立して、日本人2型糖尿病発症リスク増加と関連する可能性を明らかにした(Heianza Y, et al. Mayo Clin Proc 2012 ; 87 : 853-861)。また、男性において、飲酒頻度とは無関係に、1回飲酒量が多いことが、平均10年間の糖尿病発症リスクを増加させる可能性を示した(Heianza Y, et al. Am J Clin Nutr 2013 ; 97 : 561-568)。また、糖尿病予測スクリーニングスコアを開発し、その妥当性・有用性を検証した。採血不要の指標に加えて、過去2か月間ほどの平均血糖を反映する指標であるヘモグロビンAlcを導入することにより、糖尿病リスクスコアの予測能、予測リスク分類が大幅に改善することを示した(Heianza Y, et al. Diabetologia 2012 ; 55=3213-3223)。本検討において開発された血糖とヘモグロビンAlcを含む5年後の糖尿病発症を予測するスコアは、糖尿病ハイリスク群の特定に有用であると考えられた。
また、糖尿病患者の約半数は未診断の状態で、自分が糖尿病であることに気がついていないことも問題となっている。申請者は、約33000名を対象として、未診断糖尿病患者を発見するための評価ツールを開発し、その妥当性を包括的に検証した。これまで欧米人において開発されたツールとも比較した。その結果、本検討において開発されたスクリーニングスコアを用いた場合、現在の未診断糖尿病患者をより正確に発見できる可能性を示した(Heianza Y, et al. J Clin Endocrinol Metab 2013 ; 98 : 1051-1060)。同時に、将来の糖尿病発症リスクに関する情報が、未診断糖尿病用の初期スクリーニングスコアでも得られることが分かった。本研究で得られたスコアは、臨床の場以外で患者本人の糖尿病リスク測定に、さらに将来の糖尿病発症予防のために追跡が必要な患者の把握に役立つ可能性があると思われ

Reason

本年度は、研究の目的を達成すべく着実に研究実施計画を実行に移し、得られた結果を国内・国際学会において発表するとともに、英文原著として報告した。本研究におけるエビデンスは、日本人糖尿病のスクリーニングやその発症予測に貢献すると思われる。また、学会ガイドラインの改定のなどにも大きく貢献することが期待される。

Strategy for Future Research Activity

今後、他の日本人集団において、TOPICSで開発されたリスクスコアの妥当性をさらに検証する必要がある。現在、他の地域における健康診断・人間ドック受診者を対象とした臨床疫学研究プロジェクトを開始する準備を行っている。また、研究を進めていくうえで、問診による心理的ストレス兆候や、単身生活などの生活状況、女性における閉経の影響などの因子が耐糖能異常に与える影響についても調査する必要が示唆された。また、糖尿病発症前の肥満指標の推移について詳細についても、検討を行うべきと考えられた。それらについて検討した結果は、現在英文原著としてまとめているところであり、来年度中には研究成果の発表が期待される。
