最適な糖尿病スクリーニング法の開発とその県レベルコホートにおける実証

Research Abstract

未診断糖尿病発見リスクスコアを当てはめるべき集団を特定すべく、糖尿病関連因子についての検討をさらに発展させた。生活状況と糖尿病との関連を検討した結果、単身生活の日本人男性では、未診断糖尿病を有する可能性が高いことを認めた(Heianza Y, Diabet Med 2013 ; 30 : 1355-1359)。また、特に過体重、喫煙者や運動習慣の無い者において、単身生活は糖尿病と有意に関連しており、単身生活と未診断の糖尿病との関連は、部分的に、改善可能な他の生活習慣に影響されている可能性が示唆された。さらに、日本人女性では特に閉経後に加齢とは独立して糖尿病を有する可能性が高まる可能性を見出した(Heianza Y, et al. Diabetes Care 2013 ; 36 : 4007-4014)。閉経前女性と比較して、閉経後女性では、加齢、体格指標(BMI)や生活習慣因子とは独立して、糖尿病を有する可能性が有意に高かった。男性の有する糖尿病保有リスクには匹敵しないものの、閉経後女性では加齢とは独立して高血糖状態と関連する可能性が認められた。以上の結果より、単身生活の男性や閉経後女性などに対する糖尿病スクリーニングの働きかけが有効であると思われた。前年度に開発した糖尿病リスクスコアの妥当性を検証するため、新潟県の健康診査・人間ドック受診者のデータを活用したvalidation研究を今年度下旬より取り組み始めたところである。来年度中には研究結果が得られると思われる。
一方、糖尿病を指摘されても、多くの者が通院治療を開始せずに糖尿病を放置していることが世界的に問題となっている。申請者らがレセプトデータを活用した糖尿病スクリーニング勧果の検証を実施した結果において、糖尿病を未治療・放置することと比較して、高血糖を指摘された後すぐに通院治療を開始・継続することは、良好な血糖コントロールに寄与することが科学的に証明された。
